On Monday, February 26, 2018, pick up Application, Instructions and Information during recess or after school at the front counter of the Molokai High School school office. Limited enrollment. All application materials must be received by March 9, 2018.

*Packets will be available for pick up at the front office on February 26, 2018. You can pick up a packet before school, during recess, lunch and after school ONLY. Forms MUST be picked up from the Molokai High School office. We are not able to email forms to you.

*$10 nonrefundable payment (no cash, payable to Sam Lee)
You MUST mail all forms and payment to Mr. Lee, no later than March 9, 2018 postmark. There is a $25 returned check fee. Application will not be accepted if any forms/payment is missing.
MANDATORY MEETING
Parent Night Meeting will be on Sunday, March 18, 2018 @ 4:00pm-5:30pm in Room A103. If parent or student is absent, student will be dropped from program.
PROGRAM DATES
March 18, 2018 to May 5, 2018 (Classes will be during school days @ 3:00pm-4:30pm and behind the wheel will be during weekends and holidays).
